Referees

Before you are assigned any games in Forest Lake you need to complete and return all forms to M & D sports.

Forms Required - Proof of age (copy of birth certificate), Waiver for 2008, W-9 and FLAAA Independent Contactor Form

After all forms are turned in - Then register on M & D Sports web site and provide your availability to assignors.

There is reimbursement for certification after completion of the referee commitment.
